Position Summary: This position is responsible for periodic review of department internal control procedures, to include reviewing business processes and recommending process improvements that increase efficiency and/or accuracy of claims. This position will work with management to ensure compliance and regulations are being followed, including the Sarbanes-Oxley Act, HIPAA and Best Claims Practices.

Hours: Full Time: Monday through Friday 9 am to 6 pm. Saturday at least once a month

Location: Addus HomeCare 1641 N. Alpine Road Suites 206-208 Rockford, IL 61107. May have to travel to Sterling IL 2x monthly

Essential Duties:

  • Conducts Home Care Aide and client in-home monitoring visits to ensure compliance with client care plan
  • Delivers and reviews Client Welcome Packet and conduct in-home visits with new client
  • Completes and submits all required in-home visit forms
  • Observes, documents and reports to Service Coordinator staff, any changes in a client’s condition that are noted during a home visit. Changes include but are not limited to: Physical, mental, emotional, financial, environmental, medical, additional service needs
  • Performs necessary phone and/or in-person follow-up on reported complaints or service related issues
  • Assists with office filing, data entry, making client service monitor calls and documenting case notes
  • Assists with in-home support, training and orientation of HCA’s as needed
  • Utilizes and update the Addus HomeCare Expiring Requirements Report as directed, to ensure timely completion of required in-home visits
  • Attends all required training, both internal and external
  • Attends all required meetings, including Integrated Service meetings, if applicable
  • Available to provide in-home services to clients on a fill-in and or emergency basis, as assigned by Service Coordinator staff
  • Maintains a high degree of confidentiality at all times due to access to sensitive information
  • Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the department
  • Follows all Medicare, Medicaid, and HIPAA regulations and requirements
  • Abides by all regulations, policies, procedures and standards
  • Performs other duties as assigned

Position Requirements & Competencies:

  • High school or general education diplomas (GED), Advanced degree preferred
  • Minimum of 3 years’ experience working in the field of senior services, or related social service, including 2 years as a supervisor
  • CNA certificate and license preferred; or similar training
  • Must demonstrate strong interpersonal and communication skills
  • Represents the organization professionally at all times
  • Must be client focused and customer service driven
  • Computer experience – Microsoft Word and Excel
